A Numerical Simulation Approach to Study the Effects of MADs Embedded with Elastic Bands in OSA Treatment

Abstract: Obstructive Sleep Apnea (OSA) is a nocturnal respiratory disorder characterised by recurrent episodes of partial or total obstruction of the upper airways due to the collapse of the pharyngeal tissue. Mandibular Advancement Devices (MADs) restore regular breathing during sleep by advancing the lower jaw in a controlled way to increase the upper airway volume. However, the relaxation of muscle tone that naturally occurs during sleep induces a vertical mouth opening that impairs the efficacy of the treatment. Thus, elastic bands are recommended to keep the mouth firmly closed. Despite successfully treating OSA, inadequate evidence exists about the effects ofMADs supported by elastic bands on teeth and Periodontal Ligaments (PDLs). This study aims to develop a numerical simulation approach through the finite element method to evaluate the behaviour and the effects (displacement and stress fields) of MADs embedded with intermaxillary elastics on PDLs and teeth of patients suffering from OSA. Findings confirm the efficacy of elastics in con- trolling the mouth opening and reveal a stress increase at the anchorage regions. Thus, their use should be advised to patients who do not suffer from periodontal problems

Mixed Reality and Maxillofacial Surgery: An Interactive Twofold-Modality Application

Abstract: Using custom-made physical cutting guides in maxillofacial surgery presents several drawbacks, mainly related to design, time, and costs. Even if the literature provides several Augmented Reality (AR) solutions, they mostly display cutting planes and static assistance for repositioning. This work proposes a Mixed Reality (MR) application, deployed on Microsoft HoloLens2, to guide the surgeon in maxillofacial osteotomies and repositioning through intuitive and interactive digital content. A holographic panel allows selecting between the osteotomy and repositioning modes. With the first one, the cutting lines and the drilling points for the fixation plates are overlaid on the patient’s skull. With the second one, three different kinds of feedback simultaneously guide the surgeon toward the correct final bone placement, step-by-step, according to the real-time fragment displacement. The MR app was tested by seven inexperienced subjects using a 3D-printed skull. Satisfactory results were obtained for maxillary osteotomy (i.e., less than 1.5 mm deviations) and relocation (i.e., the accuracy is about 0.7 mm and 0.6°).

An innovative mixed reality approach for maxillofacial osteotomies and repositioning

Abstract: Craniomaxillofacial surgeries are performed using custom-made physical cutting guides and resin dental splints that present several drawbacks (e.g. time and cost required for their design and production). The literature commonly provides augmented/mixed reality (AR/MR) solutions for assisting maxillofacial osteotomies and repositioning without any interactive guide. This work proposes a new MR application, useful for osteotomy and repositioning, providing interactive, fast, and intuitive feedback to the surgeon, who is then supported in performing the bone fragment resection and replacement frame by frame. The proposed application speeds up the surgery and reduces under/overshooting errors. Moreover, the idea of integrating osteotomy and repositioning assistance in the same MR application is rarely found in the literature. It is an entirely novel approach to craniomaxillofacial surgery. The MR application has been designed with a three-button menu. The “App Start” calibrates the app, the “Osteotomy Mode” visualises the holograms of the cutting lines and drilling points, and the “Repositioning Mode” visualises the step-by-step real-time feedback to precisely support the surgeon placing the osteotomised bone fragment towards the final pre-planned position. The MR app has been developed in Unity and deployed on Microsoft HoloLens V2. A laboratory test bench was realised to validate the accuracy of the proposed MR-based approach. The validation protocol consists of two tasks to test the osteotomy and repositioning modes using a 3D-printed skull phantom. For osteotomy, the accuracy is 0.89 mm (genioplasty), 1.24 mm (maxillary osteotomy), 1.33 mm (orthognathic surgery), and 2.89 mm (mandibular angle osteotomy). For repositioning, the accuracy is 0.6 mm (anteroposterior deviation), 0.7 mm (mediolateral deviation), and 0.6° (angular deviation).

Abstract: The increase in concrete structures’ durability is a milestone to improve the sustainability of buildings and infrastructures. In order to ensure a prolonged service life, it is necessary to detect the deterioration of materials by means of monitoring systems aimed at evaluating not only the penetration of aggressive substances into concrete but also the corrosion of carbon-steel reinforcement. Therefore, proper data collection makes it possible to plan suitable restoration works which can be carried out with traditional or innovative techniques and materials. This work focuses on building heritage and it highlights the most recent findings for the conservation and restoration of reinforced concrete structures and masonry buildings.

Comparison of Three 3D Segmentation Software Tools for Hip Surgical Planning

Abstract: In hip arthroplasty, preoperative planning is fundamental to reaching a successful surgery. Nowadays, several software tools for computed tomography (CT) image processing are available. However, research studies comparing segmentation tools for hip surgery planning for patients affected by osteoarthritic diseases or osteoporotic fractures are still lacking. The present work compares three different software from the geometric, dimensional, and usability perspectives to identify the best three-dimensional (3D) modelling tool for the reconstruction of pathological femoral heads. Syngo.via Frontier (by Siemens Healthcare) is a medical image reading and post-processing software that allows low-skilled operators to produce prototypes. Materialise (by Mimics) is a commercial medical modelling software. 3D Slicer (by slicer.org) is an open-source development platform used in medical and biomedical fields. The 3D models reconstructed starting from the in vivo CT images of the pathological femoral head are compared with the geometries obtained from the laser scan of the in vitro bony specimens. The results show that Mimics and 3D Slicer are better for dimensional and geometric accuracy in the 3D reconstruction, while syngo.via Frontier is the easiest to use in the hospital setting.

Abstract: The Obstructive Sleep Apnea Syndrome (OSA) concerns episodes of complete or partial obstruction of the upper airway. Mandibular Advancement Device (MAD) is one of the most used systems for treating this syndrome. Clinicians frequently observe a combination of OSA and periodontitis. There is no research aiming to evaluate how periodontitis staging affects the overall mandibular and maxillary dental arches in the literature. Furthermore, no one has studied the combination between OSA and periodontitis and the effects of MADs on the patients in this condition. This paper aims to develop a numerical simulation approach based on FEM and evaluate the consequences (displacement and stress fields) of the periodontitis staging on PDL and teeth of patients suffering from OSA and treated with MADs. Simulations have been performed for evaluating Stage I of periodontitis. Results highlight a correlation between bone resorption and teeth displacement and periodontal ligaments stress (the higher the bone resorption, the higher the stress and displacement).

HoloLens 2 for Maxillofacial Surgery: A Preliminary Study

Abstract: Since custom-made 3D printed surgical guides for maxillofacial surgery are usually expensive, Augmented Reality (AR) can be efficiently employed to overcome the high costs. The proposed work aims to develop and test an AR application for different maxillofacial surgeries. The application consists in overlaying the cutting lines on the patient’s mandible to guide the clinician during the procedure. It has been realized in Unity and preliminary tested with HoloLens 2 and a 3D printed mandible. Seven participants performed two consecutive trials. The mandible with the obtained surgical lines has been scanned after each test to digitally reconstruct the traced lines and compare them with the surgical lines previously designed. The results allowed the preliminary analysis of the developed AR system’s accuracy and precision. Mean distances from the designed surgical guides showed good accuracy for the genioplasty (deviation error around 1.03 mm) and orthognathic surgery (deviation error around 1.27 mm), suggesting the applicability of HoloLens 2 for these kinds of surgery. On the contrary, the application was not suitable for the mandibular angle osteotomy (deviation error over 2.50 mm).

Mechanical, durability, depolluting and electrical properties of multifunctional mortars prepared with commercial or waste carbon-based fillers

Abstract: Carbon-based fillers from industrial wastes and commercial ones were compared to improve the properties of lime-based mixes. As commercial fillers, graphene nanoplatelets and activated carbon were used, whereas as industrial wastes a char obtained by the gasification of biomasses and a used foundry sand were chosen. Carbon-based wastes were found to be a good cost-effective alternative to commercial carbon based fillers to increase the compressive strength (of about 25%) and to reduce water capillary absorption (of about 50%) thanks to the paste refinement; to enhance depollution capacity (of about 25%) and increase both electrical conductivity (up to 65%) and electromagnetic shielding effectiveness (of about 6%) of the hardened compounds thanks to the carbon content.

A Procedure for Designing Custom-Made Implants for Forehead Augmentation in People Suffering from Apert Syndrome

Abstract: This paper presents a methodological procedure, based on the anatomical reconstruction and constrained deformation, to design custom-made implants for forehead augmentation in people affected by Apert syndrome, experiencing a frontal bone deficiency. According to the anthropometric theory, a cranial landmarks identification procedure was applied to retrieve, from a repository, a healthy skull, used as reference geometry for implant modelling. Then, using constrained deformation and free-form modelling techniques, it was possible to design a patient-specific implant. At last, the implant was realised using a custom mould, specially designed according to the patient’s needs to provide an accurate fit of the defect site. The design procedure was tested on a patient suffering from Apert syndrome. Three implants were virtually modelled and 3D-printed for pre-surgical evaluation. Their shapes were 3D compared with a reference one (handcrafted by a surgeon) to test the accuracy. Deviations are negligible, and the customised implant fulfilled the surgeon’s requirements.

Comparison of the Effects Caused by Three Different Mandibular Advancement Devices on the Periodontal Ligaments and Teeth for the Treatment of Osa: A Finite Element Model Study

Abstract: AIM: The purpose of this study is to compare the stress effects developed on the periodontal ligaments and teeth by three different types of mandibular advancement devices (MADs) using a finite element method (FEM) analysis. Introduction: Obstructive sleep apnea (OSA) is a disease with a high prevalence and, in recent years, the use of MADs as an alternative or support treatment to the continuous positive airway pressure (CPAP) has spread. Their use finds relative contraindications in the case of partial edentulism and severe periodontal disease. Given the widespread of periodontal problems, it is essential to know the effects that these devices cause on the periodontal ligament of the teeth. Materials and methods: Starting from the computed tomography (CT) scan of a patient’s skull, 3D reconstructions of the maxilla and mandible were implemented. Three different MADs were prepared for the patient, then 3D scanned, and lastly, coupled with the 3D models of the jaws. The devices have two different mechanics: One has a front reverse connecting rod (Orthoapnea™), and two have lateral propulsion (Somnodent™ and Herbst™). A FEM analysis was performed to calculate the stress applied on periodontal ligaments, on every single tooth and the displacement vectors that are generated by applying an advancement force on the mandible. Results: Herbst™ and Somnodent™ devices present very similar stress values, mainly concentrated on lateral teeth, but in general, the forces are very mild and distributed. The maximum stresses values are 3.27 kPa on periodontal ligaments and 287 kPa on teeth for Somnodent™ and 3.56 kPa on periodontal ligaments and 302 kPa on teeth for Herbst™. Orthoapnea™ has, instead, higher and concentrated stress values, especially in the anterior maxillary and mandibular area with 4.26 kPa and 600 kPa as maximum stress values, respectively, on periodontal ligaments and teeth. Conclusions: From the results, it is concluded that devices with a bilateral mechanism generate less and more distributed stress than an anterior connecting rod mechanism. Therefore, they may be advisable to patients with compromised periodontal conditions in the anterior area.

Abstract: This study defines a methodological procedure for the design and manufacturing of a prosthetic implant for the reconstruction of a midsagittal bony-deficiency of the skull due to the Apert congenital disorder. Conventional techniques for craniofacial defects reconstruction rely on the mirrored-image technique. When the cranial lesion extends over the midline or in case of bilateral defects, other approaches based on thin plate spline interpolation or constrained anatomical deformation are applied. The proposed method uses the anthropometric theory of cranial landmarks identification for the retrieval of a template healthy skull, useful as a guide in the successive implant design. Then, anatomical deformation of the region of interest and free-form modelling allow to get the customized shape of the implant. A full bulk and a porous implant have been provided according to the surgeon advises. The models have been 3D printed for a pre-surgical analysis and further treatment plan. They fulfilled the expectancies of the surgeon thus positive results are predictable. This methodology results to be reproducible to any other craniofacial defect spanning over the entire skull.

Abstract: Already from the beginning, 50 years ago, the first nucleus of researchers establishing the research group, was convinced that the construction sector was the best option for the valorization of industrial by-products as “secondary raw materials”. In fact, this sector is probably the largest consumer of resources and the largest waste generator, consequently it has huge environmental impact. On the other hand, construction materials affect the performance of buildings with respect to safety, health, environmental performance and energy efficiency. Manufacturing of construction products using alternative raw materials; recycling to manage construction and demolition waste; durability and environmental compatibility of materials: all these were the different and challenging fields of research that the group has faced in a continuous effort of innovation and cooperation at national and international level. The focus of the group was already perfectly in line with what is now called “Circular Economy”, which at present is considered a revolution in the way of human economic development. We are sure the group thus contributed to this revolution even before the term was in current use. We feel ready for the next 50.

Abstract: The paper represents the “state of the art” on sustainability in construction materials. In Part 1 of the paper, issues related to production, microstructures, chemical nature, engineering properties, and durability of mixtures based on binders alternative to Portland cement were presented. This second part of the paper concerns the use of traditional and innovative Portland-free lime-based mortars in the conservation of cultural heritage, and the recycling and management of wastes to reduce consumption of natural resources in the production of construction materials. The latter is one of the main concerns in terms of sustainability since nowadays more than 75% of wastes are disposed of in landfills.

Abstract: This review presents “a state of the art” report on sustainability in construction materials. The authors propose different solutions to make the concrete industry more environmentally friendly in order to reduce greenhouse gases emissions and consumption of non-renewable resources. Part 1—the present paper—focuses on the use of binders alternative to Portland cement, including sulfoaluminate cements, alkali-activated materials, and geopolymers. Part 2 will be dedicated to traditional Portland-free binders and waste management and recycling in mortar and concrete production.

Abstract: The goal of this study is to evaluate the effect of sterilization by gamma and beta radiation on the properties of four different polymeric specimens for biomedical use: a PVC pipe for connection in dialysis and heart surgery devices; a PVC band for enteral and parenteral nutrition bags, a TRIFLEX pipe (internal PE, middle PE/PVC, external PVC) for connection, a PC rigid pipe for the connection of tubes and bags for infusions. The effect of sterilization was studied by means of tensile mechanical characterization, thermo-gravimetric and morphological analysis and FT-IR spectroscopy. UV-VIS spectroscopy was used to evaluate the yellowing index, determined by radiation exposure. For each specimen, a no-sterilized one was tested for comparison.

Abstract: The shielding performance of random concrete composite samples are checked under a random excitation. It is achieved by means of a nested reverberation chamber system. Metallic fibers are randomly diffused in the concrete in order to improve shielding effectiveness from less than 10 dB up to an averaged value of 35 dB within a frequency range that includes communication system operating bands.

Abstract: Electromagnetic shielding and propagation in concrete structures are getting more and more interest in radiation hazard problems and wireless communications. The protection of sensitive environment is nowadays carried out by appropriate shielding room made of metallic walls. Even if effective they are heavy structures not adequate for the installation over existing building walls. The using of concrete composites filled by conductive elements represents a valid alternative to metallic shielded room since they can be adopted to directly build up the building walls and/or to easily plaster existing walls

Abstract: The early detection of cutaneous pigmented lesions is an important aid to the clinician in recognizing malignant melanoma. In an attempt to correlate the depth of a pigmented skin lesion with its malignant potential, phantoms able to simulate the diffuse reflectance of some lesions of different thicknesses were manufactured and tested to validate a diagnostic instrument developed in-house. Such optical skin-tissue phantoms may be useful for accelerating and optimizing the diagnosis of suspicious lesions of the skin. In fact, benign melanocytic lesions are different in terms of their diffuse reflectance from melanoma. The diffuse reflectance of pigmented skin lesions depends on the amount and distribution of the absorbing/diffusing chromophores embedded in the skin layers. The basic phantom material is a PVA hydrogel in which appropriate amounts of optical scatter are added extraneously at the time of formation to achieve tunability of the optical properties. Liquid Indian ink is used to simulate melanin and all the other chromophores. Slabs were prepared to mimic lesions of different depths. The optical properties of the tissue phantoms were determined in the visible and near-infrared spectral ranges using a noninvasive instrument made from a purpose-modified digital camera. The measured reflectance was correlated with the depth of the lesion both in a Monte Carlo simulation environment and in a laboratory experiment.

A method for Performance Evaluation of RE/RP Systems in Dentistry

Abstract: Purpose: The aim of this study is the investigation of a method in order to compare the scanning and reproducing accuracy of highly shaped objects like plaster casts used in dentistry. Design/Methodology/Approach: Theoretical considerations on errors introduced by the scanning systems and subsequent point clouds data elaboration have led to a method to estimate the accuracy of the whole process. Suitable indices have been chosen and computed at each stage. As a final result the overall chain, scanning and reproducing systems, can be assessed. In order to validate the proposed method casts have been scanned by means of commercial systems and then reproduced by using different RP technologies, materials and parameters. Error indices have been computed and reported. Findings: Since it is not possible to define reliable and meaningful reference models for nonstandard shapes, an absolute accuracy value for the scanning process cannot be stated. Anyway the proposed method thanks to relative performance indices allows the comparison of different acquisition systems and the evaluation of the most performing manufacturing chain. Practical implications: The study provides a method to assess the relative performance between commercial systems both in scanning and reproducing stage. Originality/Value: In literature some studies on the accuracy of scanning devices have been found but they are based on standard geometrical features. In this paper the problem of complex shapes in absence of reference model is addressed instead.

Direct fabrication through electron beam melting technology of custom cranial implants designed in a PHANToM-based haptic environment

Abstract: Repairing critical human skull injuries requires the production and use of customized cranial implants and involves the integration of computer aided design and manufacturing (CAD and CAM). The main causes for large cranial defects are trauma, cranial tumors, infected craniotomy bone flaps and external neurosurgical decompression. The success of reconstructive cranial surgery depends upon: the preoperative evaluation of the defect, the design and manufacturing of the implant, and the skill of the operating surgeon. Cranial implant design is usually carried out manually using CAD although this process is very time-consuming and the quality of the end product depends wholly upon the skill of the operator. This paper presents an alternative automated method for the design of custom-made cranial plates in a PHANToM-based haptic environment, and their direct fabrication in biocompatible metal using electron beam melting (EBM) technology.

Abstract: The development of an optical skin-tissue phantom can be useful in accelerating and optimizing the diagnosis of suspected lesions of the skin tissue. In fact, benign melanocytic lesions are different, in terms of diffuse reflectance, from that of melanoma. Diffuse reflectance of skin pigmented lesions depends on the amount and distribution of the absorbing/diffusing cromophores, mainly haemoglobin and melanin, embedded in the skin layers. In an attempt to correlate the thickness to the dangerousness of the lesion, phantoms able to simulate the diffuse reflectance of some lesions with different thickness have been developed and used to validate an inhouse developed survey instrument potentially useful to the dermatologist to evaluate the dangerousness of a pigmented skin tissue lesion. The basic material for lesions and skin-like phantoms is a poly(vinyl alcohol) - PVA transparent hydrogel in which appropriate amounts of optical scatterers have been added extraneously at the time of formation, to achieve the tunability of optical properties. Abstract: This paper presents a shielding effectiveness evaluation of densified with small particle (DSP) cement composite at microwave frequencies by means of an improved nested reverberation chamber (NRC) technique, more faster than the classical one. In fact, the mechanical stirring inside the inner chamber was replaced by the frequency stirring method (FS) and only one scattering parameter was acquired to estimate both empty and loaded aperture. By this method, the stirrer movement becomes unnecessary. A software was used to manage the measurement and the post-processing operations.
